I am sending this email to document the incident that we discussed on the phone this morning at 9:26 am.

Last week, it was brought to my attention that our GDS-Washing Hauling Company picked up a roll-off
container from one of our customer locations on August 5, 2016, and he did not have proper
documentation for the particular waste in the container. At the time that the container was picked up, the
driver thought that the container only contained construction and demolition (C&D) debris (this was his
assumption after observing only C&D type waste in the top of the container). However, it was later
determined that the container also contained non-friable asbestos. After the container was picked up at
the customer location, the container was transported to our Beaufort Transfer Station (DEQ DWM Permit
#0703) and dumped onto the tipping floor. All of the waste that was inside the container and dumped
onto the tipping floor was loaded into one transfer trailer and shipped to the East Carolina Landfill (DEQ
DWM Permit #0803), where the waste was placed and compacted in the working face.

Since discovery of this issue, we have completed the proper asbestos waste shipment record, including
all signatures. In addition, the asbestos load has been identified by East Carolina Landfill personnel and
has been recorded in the Operating Record as disposal of asbestos waste.

To prevent a future occurrence of this incident, we will review this incident with all of our drivers at the
GDS – Washington Hauling company and add a note on their individual route sheets with special
handling instructions for similar types of waste loads.
