OBSERVED VIOLATIONS:
1. An incorrect permit number is displayed on this vehicle. Also the firm name is split between the truck door and
the tank sides. Display the correct permit number (NCS-00076) & the complete firm name (Don Jordan’s Septic
Tank Cleaning & Portable Toilet Rental) on both sides of this vehicle, (no letters or numbers less than three
inches in height) per Rule .0844(b). A follow up inspection will be performed the week of August 22nd. The
town name & phone number were correct on both sides of this vehicle.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle Tag#YA132675 to be added to the firm permit. No additional fee required for this truck.
2. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted.
3. The other permitted vehicles were not inspected & the truck logs were not reviewed at this time.
