OBSERVED VIOLATIONS:
1. The required town name, as per Rule .0844(b), is missing on both sides of this vehicle. Correctly display
“Pikeville” on both sides of the vehicle, no letters less than three inches.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected second vehicle to be added to the firm permit. Tag#YA130941, VIN 1HTWHAAR655143478, 3300
gallons. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were
noted.
2. When the lettering is corrected please submit a photo of the “town name”. When the Raleigh Central office
receives the photo and the additional $250 fee then this vehicle may be used for septage pump jobs.
3. The other permitted vehicle was not inspected and truck logs were not reviewed at this time.
