EXPIRES: 10/12/15 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: An observed violation to Rule .0838(a)(18) was noted on 10/30/14 for not having the annual ryegrass planted yet. Rows of rye were observed today where the rye had been drilled into the bermudagrass. See picture on next page. OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1) Required signage present along access road at the pond before you get to the site. Some vegetation needs to be cleared from the signs. 2) Access road is well maintained. 3) A few markers were down. Need to get markers back up. 4) No septage discharges noticed outside of the permitted boundary of the field. 5) No application issues noticed today. 6) Rows of rye observed from where the rye was drilled into the bermudagrass. Some weed control needed. See pictures below. 7) No application at time of inspection. pH not checked. 8) No objectionable odors noticed today. 9) No issues noted with setback distances. 10) Application for permit renewal received on 08/03/15. Rows of rye observed. Marker down at red arrow. Broadleaf weeds present in the field.
