Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be
subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Appropriate signage present as you enter site.
2. Access road well maintained. 3. Please place field boundary markers along the south-western side of Field 4 where I have placed the orange
flags.
4. Field was wet. Limited check of fields. No discharge noticed outside of field boundaries. 5. Limited check of fields due to wetness. No application issues noticed.
6. No small grain noticed in Fields 1 and 3. Fields 2 and 4 have fescue present.
7. pH not checked. No application at time of inspection. 8. No objectionable odors noticed today.
9. No change noticed that would affect setback distance.
10. Application for permit renewal has been received. Working with Mr. Will Mann, Fishing Creek Soil and Water Conservation District on updating the Nutrient Management Plan.
11. Please place markers where the orange flags were set for Field 4. No small grain observed.
