STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 06/09/15.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) The required signage present as you enter the site.
2) The access road is well maintained.
3) Some markers were down. Discussed with Blake the markers that need to placed back up. Blake had
already put some of the markers back up.
4) No evidence of any recent applications. Blake said that they have been taken the septage to the WWTP.
No septage discharges noticed outside of the permitted boundaries for the fields.
5) No application issues noted. No spot dumps or thick applications noticed.
6) The rye had been overseeded (drilled) within the bermudagrass and was up on both fields.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) No change noticed that would affect any of the setback distances.
10) Records not checked.
11) No violations noticed. I collected composite soil samples from each of the two fields.
