ADDITIONAL COMMENTS
1. The transmission Line Phases 1 & 2 (aka the Twin Sisters) and Access Roads Structural Fills were
constructed in accordance with the Allen Ash Reuse Permit #WQ0003255.
2. According to the reporting for the Allen Ash Reuse Permit #WQ0003255, the Duke Energy Corporation –
Allen Plant Steam Station completed construction of the structural fill projects in 1995. A total of 172,722
cubic yards were utilized for these projects.
3. The transmission Line Phases 1 & 2 (aka the Twin Sisters) are located to the west of the Retired Ash Basin
(RAB) Landfill (permit # 3612-INDUS-2008). The eastern structural fill has an equipment laydown yard on
the northern portion which can be used in the event of large scale mobilizations. An additional laydown yard
is planned for the southern portion of the eastern structural fill. This laydown yard will be accessed by a
separate access road to reduce vehicular conflicts.
4. Coal combustion by-products utilized on exterior slopes of the structural fill facility shall not have a slope
greater than a 3:1 ratio as per 15A NCAC 13B .1705(l).
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

5. Final cover is being well maintained. Native vegetation is covering the tops and side slopes of the structural
fills. The laydown yard area is covered in compacted stone. No erosion or ponded water was observed
during the inspection.
6. During the inspection in June 2015, Section staff observed that a portion of station access road southwest of the
station was constructed of uncovered fly ash causing vertical plumes of dust when the road was used by
vehicles. The ash on this road has been covered to prevent dust. No dust was observed coming from this access
road during the June 10, 2016 inspection.
