Matthews Jr. Address: PO Box 233 – Farmville, NC 27828 Telephone: 252-714-7890 (Virginia) Email address: matthewsseptictank@yahoo.com PARTICIPANTS: Chester Cobb STATUS OF PERMIT: Expired EXPIRES: 10/12/15 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: No violations noted on the previous inspection conducted on 06/09/15. OBSERVED VIOLATIONS: 1) Permit expired on 10/12/15 without an application for permit renewal being received by this office. This is a violation of Rule .0835 (g) that states “Applications for renewal permits shall be submitted to the Division at least 90 days prior to the expiration date of the permit….” 2) “No Trespassing” sign not present as you enter the site. This is a violation of Rule .0838 (a) (2) that states “Each site shall be posted with "NO TRESPASSING" signs. Access roads or paths crossing or leading to the disposal area shall be posted "NO TRESPASSING" and a legible sign of at least two feet by two feet stating "SEPTAGE LAND APPLICATION SITE" shall be maintained at each entrance to the land application area.” The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1) A sign was labeled “Septage land Application Site” as you enter the site. Need a “No Trespassing” sign also at the entrance to the site. See picture below. 2) The access road is in good shape. 3) Markers present. 4) All septage discharges appeared to be within the permitted boundaries of the field. 5) No application issues noted today. No spot dumps or thick applications noticed. 6) The rye had been overseeded into the bermudagrass and was up. The rye looked good. 7) No application at time of inspection. pH not checked. 8) No objectionable odors noticed today. 9) No change noticed that would affect setback distances. 10) Permit expired on 10/12/15. No application for permit renewal has been mailed to the office. I met with Curtis and Virginia Matthews on 11/18/15 to talk about this permit. “No Trespassing” sign needed with Septage Land Application Site sign.
