with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) The required signage was present before you get to the site. The vegetation had been cut from around the
signs so that they could easily be seen. Thank you. See picture below.
2) The access road is in good shape.
3) The field boundary markers were present and up.
4) All septage discharges appeared to be within the permitted boundaries of the two fields.
5) No recent applications noticed. No thick applications or spot dumps noticed.
6) Bermudagrass was present. Broadleaf weed control is needed. See picture below.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) No change noticed that would affect setback distances.
10) Thanks for removing the vegetation from around the signs. Broadleaf weed control is needed on the fields.
