PURPOSE OF SITE VISIT: Follow-up
STATUS OF PAST NOTED VIOLATIONS:
Site had been modified was noted on the previous inspection conducted on 10/09/14.
OBSERVED VIOLATIONS:
1) Site had been modified since the permit was issued on 09/30/13. A screening box and three additional plastic
tanks have been added. This is a violation of Rule .0836 (r) that states “Applications for permit modifications
shall be required for changes in: (4) Size and number of treatment or storage structures.”
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) “No Trespassing” sign present as you enter the site.
2) Area around the tanks and facility is well maintained.
3) No issues noticed with the tanks present. No leaks noticed. Inspected the three additional 6,500 gallons
plastic tanks. No issues noticed with the three additional tanks. No issues noticed with the added
screening box. While no visual issues noted, an application for permit modification, updated drawings,
and updated operation manual needs to be submitted.
4) No septage spills noticed.
5) No objectionable odors or vector issues noticed.
6) With the added tanks, the setback distance to the well on the current drawing comes into effect because of
double setbacks with the added volume now over 25,000 gallons. David has abandoned the well. I need
documentation that the well was properly closed out.
7) Records not checked.
8) An application for permit modification, updated drawings, and updated operation manual needs to be
submitted as soon as possible. Also, the documentation for the closure of the well needs to be submitted.
