STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 06/11/15.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) The required signage was present at the access roads to the site.
2) The access road is ok. Some potholes are getting bigger and deeper.
3) Field markers present on Fields 1 and 2. No markers present for Field 3. Be sure to have the markers up
when the site is in use.
4) No septage discharges noticed outside of the permitted field boundaries.
5) No application issues noticed. No spot dumps or thick applications noticed.
6) Soybeans present on the fields. Soybeans will need to be harvested and the small grain crop planted if you
plan to use the site this fall / winter.
7) The pH of septage was not checked. No application at time of inspection.
8) No objectionable odors noticed today.
9) No change noticed that would affect setback distances.
10) Records not checked.
11) I collected composite soil samples from each of the three fields.
