with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:

1) Signage present as you arrive at Field 2. Field 2 is the first field that you get to.
2) Access road was in good shape.
3) Markers present.
4) All septage discharges appeared to be within the permitted boundaries.
5) No spot dumps or thick applications noticed today.
6) Weed control needed. Areas of the fields had been replanted.
7) No application at time of inspection. pH not checked.
8) No objectionable odors noticed today.
9) No change noticed that would affect any of the setback distances.
10) No violations noted today. Met with Maurice and Rosalee concerning permit. Mr. Cleveland Mitchell had
passed away. Need to get permit in Rosalee and Maurice names.
