or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected second vehicle to be added to the firm permit. This truck was permitted in the past. The Raleigh
Central office has received the additional fee to add the second truck to the firm permit. Tag#YA121509, 2000
gallon capacity, VIN 1HT4DUXP4GHA33077.
2. The tank appears structurally sound and securely attached.
3. Outlet covers were on and no drips/leaks were noted.
4. The lettering is correct on the passenger side. On the driver side the correct information is displayed
however the firm name, town name and phone number are damaged and need to be repainted/touched
up so that it is clearly visible. Please repair the lettering on the driver side of the tank within 15days and
send a picture of the repaired lettering to my email address [e-mail].
5. Did not inspect the other permitted vehicle or review truck logs at this time.
