Weldon, NC 27890 GPS COORDINATES: N: 36.34404 E: -77.64627 FACILITY CONTACT NAME AND PHONE NUMBER: Name: James Greene Jr. Telephone: 252-536-2758 FACILITY CONTACT ADDRESS: Same as above PARTICIPANTS: Chester Cobb, DWM STATUS OF PERMIT: In Process of Closing Out EXPIRES: 7/9/2013 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: A Notice of Violation was issued on 2/24/15 for the violations noted on 1-15-15. The violations still have not been resolved. See observed violations below. OBSERVED VIOLATIONS: 1) Septage was still on the ground behind the storage tank farthest from the fields. See picture below. This is a violation of Rule .0841 (e)(1) that states "Each detention and treatment facility shall be designed, constructed, and maintained in such a manner as to: Prevent leaks or the flow of septage out of the facility into the seasonally high water table, onto the ground surface, or into any surface waters." The septage on the ground by this storage tank needs to be cleaned up, treated with hydrated lime, and spread out on one of the fields that were permitted under SLAS-42-06 or cleaned up by a permitted septage firm as outlined in the Notice of Violation issued on February 24, 2015. See picture below. This is a violation of Rule .0841(r) that states “Septage shall not be stored in a detention or treatment facility for more than six months.” See picture below. Septage needs to be removed from the tank, treated with hydrated lime, and spread out on one of the fields that were permitted under SLAS-42-06 or cleaned up by a permitted septage firm as outlined in the Notice of Violation issued on February 24, 2015. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
