Notice of Violation issued on 4/20/15 for violation to N.C.G.S. 130A-291.1(c) for failing to obtain Septage
Management Firm permit prior to transporting septage in NC. A check for $800 was received on 5/11/15 for the
double fee that was assessed for transporting septage in NC before obtaining a permit. Also on 5/11/15, Mr. Artis response to the Notice of Violation was received.

OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. I inspected the following pump trucks:

2. The Firm name, town, phone number, and permit number was on both sides of the two pump trucks. We
removed “Cleaning” from the Firm name on the application since it was not on the trucks. We added the phone number of 538-1480 to the application since it was not on the application. The lower case letters for
the town name, Suffolk VA, were less than 3 inches in height. All other letters were at least 3inches in
height. Mr. Artis sent me pictures on 5/6/15 and 5/8/15 showing that the letters for the town were 3 inches in height (all letters the same size) and that the area code had been added to the phone number. See pictures
below. 3. The tanks for both trucks were made of steel and appeared to be structurally sound.
4. The tanks were bolted to the frames of both trucks.
5. The valves appeared to be in working order. Caps present on the hose ports. Access ports appeared to be in good repair and have proper fitting lids.
6. No leaks noticed.
7. Preoperational inspection of trucks. Truck logs not inspected.

9. Upon review of the pictures submitted by Mr. Artis the two pump trucks past inspection. The permit
for NCS-01321 was issued on 5/12/15.
