The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain
compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations,
Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a
solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1. LETTERING All required lettering on both sides of the pump truck. All required
lettering at least 3 inches in height. Pumper vehicle (s) properly lettered
meeting rule requirements for information
2. TANK (S) Tank made of metal and appeared to be structurally sound.
The tank was structurally sound and
constructed out of approved materials
3. ATTACHMENT Tank bolted to the frame of the truck.

4. VALVES, PORTS, and
CAPS
Valves appeared to be in proper working order. No issues noticed with any of the access ports. Caps present on ports.

5. LEAK No leaks noticed.
No evidence of any leaks from the tank,
valves, and/or access ports.
6. TRUCK LOGS Since this was a pre-operational inspection, no truck logs to inspect. Quickly reviewed the requirements for the truck logs. Logs contain the required information and are up to date
OTHER COMMENTS:
No issues noticed to prevent permit from being issued. Septage Management Firm Permit, NCS-01309, was issued on
1/15/15. Permit has an expiration date of 12/31/15.
