Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) 400 waste / 200 water 1) I inspected a slide in tank unit mounted on a trailer. The tank was labeled on both side with the required information. All lettering was at least 3 inches in height. See pictures below. 2) The tank was made of metal and appeared to be structurally sound. 3) The tank was bolted to the frame of the trailer. The bolts appeared to be secured. 4) The valves and access ports appeared to be in proper working order. Cap present on the port used to discharge the septage from the tank. 5) No leaks noticed. 6) Truck logs not inspected today. The tank was lettered on both sides. Tank mounted on dual axle trailer. Kevin provided an email received from Ms. Elizabeth Patterson with Division of Waste Management on January 6, 2015 stating that she had “spoken with the treatment plant and let them know you are properly permitted and have submitted your renewal for 2015 which is being processed. They emailed over a copy of the authorization and should let your truck in now. Copy of email attached. I was able to find a copy of the wastewater treatment plant authorization. Kevin found the certificate documenting that he had obtained the 4 hours of annual training for septage management firm on December 13, 2014 in Raleigh, NC.
