Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible
parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a
solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste
Management Act and Rules.

ADDITIONAL COMMENTS

1. I inspected the following pump truck:

TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal)
EH 2385 1FDUF5GTXFEB2119 600 waste / 300 water

Pump truck with license plate# BP 3817 and VIN# 1FDAF56PX5EC28209 has been sold.

2. All required lettering was present on both sides of the vehicle and all letters were at least 3 inches in height.
3. New pump truck and tank. The pump tank appeared to be structurally sound.
4. The tank was bolted to the frame of the truck.
5. The valves and access ports appeared to be in working order.
6. No leaks noticed today.
7. Caps on the ports used to collect and discharge septage.
8. Records not checked today.
9. Truck passes inspection.
