ADDITIONAL COMMENTS
1. The closed Pine Hall Ash Landfill was utilized primarily for the disposal of ash generated by the Belews Creek
Steam Station and ceased receiving waste February, 2004.
2. Initial closure plan approval for the approximately 37.9 acre landfill was issued by the Solid Waste Section on
December 7, 2007. Closure and cap construction was conducted April-December 2008. Additional approvals
have been issued for landfill cap repairs/refit and assessment monitoring since closure was complete.
3. On October 1, 2015, Duke Energy reported to NCDEQ that two ash boils were discovered that morning on the
west slope of the landfill. After an investigation into the causes, on February 25, 2016, Duke Energy proposed a
Construction Quality Assurance Plan to modify the cover system. The plan was approved by NCDEQ on
February 26, 2016.

4. The groundwater/surface water monitoring report for the sampling event conducted October 26, 2015 was
reviewed during the inspection.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

5. Monitoring wells with previously noted concerns were observed during this inspection. The following

6. Permanent edge-of-waste markers are in place and being maintained.
7. Facility access roads and perimeter roads are well maintained and of all-weather construction.
8. Sedimentation and erosion control features are in place and well maintained, no on-site or off-site erosion
observed.
9. Established vegetative cover observed over the entire landfill. Periodic mowing of vegetative growth is
conducted for the facility and landfill cap.
