Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS On February 22, 2016, Susan Heim and John Patrone met with Mike Phillips, facility operator, to conduct a comprehensive inspection of the Veach LCID Landfill, located at 4261 Old Thomasville Road, Davidson County. 1. The facility is a land clearing and inert debris (LCID) landfill. 2. The facility occupies ~ 9 acres of an 82-acre tract. 3. The facility entrance is located on Old Thomasville Road between Spry Road and Crotts Lane. 4. The facility has been in operation for ~1.5 years, and has been inspected on two previous occasions. 5. The facility has been approved as a Temporary Disaster Debris Site (DS29-06); the site was not operational at the time of this inspection. 6. The facility operates by appointment only, and is permitted to receive waste generated within Davidson and Forsyth Counties. 7. The facility is secured by a locked gate and is posted with proper signage. 8. The facility is permitted to receive land clearing waste; yard trash; untreated and unpainted wood; uncontaminated soil; inert debris, such as unpainted rock, brick, concrete and concrete block; and asphalt, in accordance with NCGS 130-294(m). 9. As stated in the facility annual report, dated 8/15/15, for the period July 1, 2014 through June 30, 2015, the approximate amount of waste accepted at the facility for disposal was 675 tons, or ~15 tons per week. 10. The facility mines soil, and the mined area is being used as the current landfill area. 11. An inspection of the facility was conducted by Division of Energy, Mineral and Land Resources (DEMLR) in November 2015. 12. Vegetative debris and inert materials accepted for disposal are placed adjacent to the working face, spread out for drying if necessary, and pushed over onto the working face of the landfill, located currently to the west of the entrance drive. 13. The facility crushes concrete, stores clean soil and soil mixed with inert debris, and accepts land clearing debris to be landfilled at the site. 14. Screening equipment is planned to be installed adjacent to the crushing equipment east of the entrance drive for use in separating soil from inert materials, according to Mr. Phillips. 15. A scale is onsite now awaiting installation. 16. A new concrete driveway entrance has been installed, with a heavy metal grate at the end of the concrete pad that is designed to remove soil and debris from truck wheels prior to exiting the site. 17. Mr. Phillips indicated that the pit under the metal grate will be piped to direct water to the sediment pond, thereby providing adequate drainage and preventing standing water. 18. Clean soil for use as cover is stored adjacent to the working face. 19. Ensure that adequate cover is applied at least monthly (every 30 days) or when the active landfill area reaches one acre in size, whichever occurs first, as provided for in 15A NCAC 13B .0566(4). 20. Ensure that waste being landfilled is restricted to the smallest area feasible and compacted as densely as practical into cells, as provided for in 15A NCAC 13B .0566(3). 21. Soil mixed with inert materials is stored on the east side of the entrance drive prior to segregating materials and screening. Stockpiles are graded to prevent ponding at the base. 22. Ensure that excavation, grading and fill material side slopes do not exceed a ratio of 3:1. 23. Open burning of land clearing waste is prohibited at the site. 24. Ensure permanent markers that accurately designate the waste disposal boundary are installed and maintained. 25. Ensure that the mined area and base of the working face are properly graded so as not to collect water. 26. Ensure that the site is policed throughout the work day and that prohibited materials, including trash, tires, etc., are segregated and stored, if necessary, under cover prior to removal to a permitted disposal facility. 27. It is suggested that areas of the site be graded and designated for the staging of materials requiring separation or processing as they arrive onsite. 28. The PTO expiration date is June 28, 2018; the permit renewal application due date is December 28, 2017. Please contact me if you have any questions or concerns regarding this inspection report.
