OBSERVED VIOLATIONS:
1. 15A NCAC 13B .0505 OPERATIONAL REQUIREMENTS FOR SANITARY LANDFILLS
Any person who maintains or operates a sanitary landfill site shall maintain and operate the site in conformance
with the following practices, unless otherwise specified in the permit:
(4) Erosion Control Requirements
(b) Adequate erosion control measures shall be practiced to prevent excessive on-site erosion.
During the June 15, 2016 inspection, exposed asbestos bags were observed in several areas in the Asbestos Containing
Materials (ACM) disposal area. It appeared that the ACM had been covered with adequate soil cover at time of disposal,

however excessive rill erosion and lack of groundcover caused the material to become exposed.
Within ten (10) days Evergreen Packaging Inc. and Blue Ridge Paper Products Inc. must repair the rill erosion, replace the
18 inches of cover soil as specified in the “Operations & Maintenance Manual Landfill No.6, section 2.9.3.2 Site Security
and Maintenance.” Seed and mulch the area with the intent to establish a groundcover sufficient to restrain erosion in the
Asbestos Containing Material (ACM) area.

ADDITIONAL COMMENTS
All photos were taken by Lee Hill on June 15, 2016.
The facility is permitted to receive solid waste Generated by Blue Ridge Paper Products.
The facility is permitted to receive approximately 450,000 cubic yards of material per year.
Records Review
Tonnage records, permit, operations plan, and sampling reports are kept at the Canton Mill Office.
The permit and operations plan were available upon request.
Monthly landfill haul data was reviewed from January 2016 to May 2016.
The facility haul data shows the facility is landfilling sludge, lime mud, fly ash, ciders and wood waste.
Haul data is broken down by cubic yards, tonnage and truckloads
The facility disposed of 194,612 cubic yards during this period.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

Haul data indicates the facility landfilled 461,738 cubic yards during 2015.
The leachate collection system is connected to three leachate ponds which is piped to the waste water treatment
facility at the plant.
Leachate pump records were reviewed from January 2016 to May 2016.
o January: 7,122,015 gallons
o February: 4,289,570 gallons
o March: 2,768,700 gallons
o April: 1,969,890 gallons
o May: 1,902,105 gallons
Financial Assurance Approval letter dated December 14, 2015 was available for review.
May 2015 and November 2015 semi-annual groundwater reports were reviewed. Sampling was conducted on
May 11-13 and November 2-5 in accordance with table 2.1 of appendix C in the Environmental Plan located in
the Operations Plan.
Landfill Gas sampling was conducted on 1/29/15, 5/28/15, 9/1/15, 10/22/15 and 3/22/16.
Landfill training records were available for review. Seventeen staff members received annual landfill training
on March 23, 2016.
Quarterly landfill inspection records were reviewed. The last quarterly inspection occurred on 4/1/2016.
Operations Inspection of Landfill Unit (4406)
The landfill entrance was secured with a gate and proper signage was observed.
The facility roads were constructed of pavement and gravel.
The facility is currently filling in Landfill No. 6 Area D –North.
Multiple working faces were being utilized at the time of inspection depending on the consistency of the fill
material.
No unapproved material was observed at the time of inspection.
Landfill side slopes were being maintained at the time of inspection.
Edge of Waste Markers were observed on all fill areas on the landfill.
Three leachate ponds were observed at the time of inspection and the liners appeared to be in good condition.
No issues were observed on the caps of the closed landfill cells were observed.
Solar panels are located on B-Lower section of the closed landfill. Facility staff stated that it is the
responsibility of FLS Energy to maintain the cap on this portion of landfill.
Major rill erosion was observed in the Asbestos Containing Material (ACM) disposal Area. It appeared that
proper soil coverage was over waste at the time of disposal however cover had been eroded. Please see
observed violations section of this report.
Four locations of exposed asbestos waste bags were observed during the inspection. This waste must be
covered immediately. Please see observed violation of this report.
Facility staff stated that the ACM area is not used on a frequent basis. Groundcover sufficient to restrain
erosion needs to established to prevent further erosion issues in this area.
On June 16, 2016 facility staff sent pictures of repairs to the cover soil that were made in the Asbestos
Containing Material (ACM) area. Please ensure that 18 inches of cover soil is replaced in the eroded areas
as specified in the “Operations & Maintenance Manual Landfill No.6, section 2.9.3.2 Site Security and
Maintenance.”
Groundcover establishment in the Asbestos Containing Material (ACM) area will be evaluated in subsequent
inspections of the facility.
