SEE BACK FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 Upon review of the treatment logs for permit renewal it was noted that information was missing - such as how much processed material (liquid & solids) were being removed from the facility & on what days did this occur. Also company names for daily discharge events were not being recorded. Enclosed is an example treatment log. Please revise your facility treatment log to include all the required information per Rule .0841(v). Please send me a copy of the revised treatment log for future reference. ADDITIONAL COMMENTS 1. Permit submittal was reviewed. Walked thru process/facility in regards to permit renewal. 2. The above ground tanks appear structurally sound. 3. The approved setbacks are being met. No physical change in facility. 4. The required 5’ cleared radius (free of excess vegetation) was present around the facility at this time. Please maintain in the coming warm months. 5. The access road is adequate and the required sign is posted. 6. No septage spills noted on the ground surface, but the inside of the building & the containment area need additional cleanup and an improvement in procedures to promote more sanitary methods.
