Per our discussion this afternoon, I am attaching several documents to assist in completing the Corrective Action 
Application for the Town of Wake Forest closed N. White St. Landfill.  
Also attached is a copy of the NC Solid Waste Section’s Monitored Natural Attenuation Guidance document and the 
April 2001 Post Closure Permit and original December 11, 1980 Solid Waste Management Permit for  the Town of Wake 
Forest closed North White Street Landfill. 
Please review the documents and let me know if you have additional questions.

Christine,  

Last week we submitted the signed Pre‐CAP Application for North White Street LF to Elizabeth, but she pointed out we 
didn’t furnish the required attachments listed on page 1 of the Application. Our apologies for missing this – I am sending 
you this email so that we can provide the needed documentation. 

I have a few questions regarding the required attachments: 

What is required specifically for Item 1 (signed resolution/proclamation)?  
We can supply the site map for Item 2 (which is in each biannual sampling report).  
As for Item 3, if we are proposing MNA, I’m not sure there are conceptual draft documents to provide. 
Would NCDEQ be able to provide the registrations and permits for this landfill (Items 4‐6)? We do not have these on file. 
Finally, I would imagine Item 7 is not applicable.
