administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspection of vehicle for first firm permit. Tag#FJ-8785, VIN 1GDM7H1J2RJ507200, 800 gallon waste
capacity. The tank appears structurally sound and securely attached. Outlet covers were on and no drips/leaks
were noted. The lettering is correct on both sides of the tank.
2. I informed the Raleigh Central office that the truck is in compliance. No septage pump jobs shall be
performed until the firm permit has been issued/signed.
3. Enclosed is an example truck log. After the permit is issued and pump jobs commence, please begin
maintaining the required information. Ensure each porta john route is listed on the log.
