ADDITIONAL COMMENTS
1. All Photos were taken by Troy Harrison on May 10, 2016.
2. On May 5, 2016, a complaint was received from Mr. Shawn Harber, Fire Captain, City of Winston-Salem via
telephone concerning Carolina Resource Recovery (CRR). He stated that the smell was horrible this morning.
3. I contacted Michael Settle with Forsyth County Environmental Assistance (Forsyth County) who conducted an
investigation. He later called me back and stated that odors were objectionable at the Fire Station No. 5 located
at 771 Palmer Lane. Mr. Settle also met with personnel at CRR.
4. A meeting was scheduled at CRR with myself, Forsyth County and CRR on May 10, 2016 at 10:00 AM.
5. I arrived in the area at approximately 9:32 AM and parked on a location (Site 1) at Palmer Lane directly across
from the Carolina Resource Recovery Facility (CRR) and near the intersection with Rothrock Street. No
objectionable odors were observed.

6. At 3:39 AM, I began driving around the area on Rothrock Street, Cassell Street and Clemmonsville Road,
pulling off the roads and stopping several times. No objectionable odors were observed.
7. The permitted facility consisted of: One 10,000 gallon tank, one 10,000 raw material bin and three 25,000
gallon tanks for a total storage capacity of 95,000 gallons for the storage of untreated and treated septage.
8. The facility is permitted for the storage and treatment of grease trappings only.
9. The facility is surrounded by chain-length-fence with multiple gates to prevent unauthorized access.
10. No trespassing signs were provided on the exterior side of the chain-length-fence.
11. All waste treatment was maintained to the interior of the building.
12. Solid waste generated as a result of the treatment process is loaded via screw conveyors
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

13. The truck off-loading area was free of residues and well maintained. No vehicles were observed being
unloaded. CRR staff stated that approximately 7 to 13 trucks are off loaded each day.

14. The wet misting system, located on the southern side of the building was not in operation due to problems with
the operation. This has been replaced by another system was in place using what appeared to be a dry evaporative
system that had a citrus smell as a masking agent.
15. The exact cause of the odors on May 5, 2016 could not be determined, however, that was an unusually cold day.
The production of odors vary depending on the load; with variables depending on age and type of waste. CRR
staff stated that after the complaints on 5/5/2016, the sump below the scrubber is cleaned every day. An
updated Odor and Vector Control Plan was available and contained new updated information and procedures
outlined in red.
