The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The permitted facility currently consists of: One above ground 10,000 gallon tank, four above ground 3,000
gallon tanks and two buried 900 gallon tanks for the storage of domestic septage and grease trap pumpings.
Currently, this facility is only being utilized for septage detention and the Division shall be notified and
approval granted before any treatment process is implemented.
2. The permit application submitted on 11/16/2015 does not list the two buried 900 gallon tanks, however they
were observed in use. An e-mail from Mr. Blankenship on May 2, 2012 request that these tanks be included in
the permit.
3. The 10,000 gallon tank and two of the above ground 3,000 gallon tanks located in the rear of the building were
not in use, according to Mr. Blankenship.
4. “No Trespassing” signs were posted on the exterior of the building at the time of inspection.
5. No objectionable odors were observed outside of the facility.
6. It appeared that the setback requirements were being maintained as originally permitted.
7. The permitted tanks appeared to be structurally sound and no leaks were observed.
8. It appeared that facility operations were being carried out as detailed in the approved Permitting and Operations
Manual for the facility.
9. No spills were observed on the ground outside of the building.
10. Only waste from Able Septic Tank Service is accepted at this facility.
11. Only septage detention is done at this facility. The Division must be notified and approval granted before any
treatment process is granted.
