STATUS OF PERMIT:
4302-LCID-1998 Permit to Operate issued May 26, 2015. Permit will expire on September 24, 2026.
Life of Site Permits issued for 4302-CDLF-1998 and 4302-CDLF-2018 on July 23, 2020. The Life of Site Permits will
expire on March 2, 2058.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
Facility Inspection
1. The facility is fenced and secured by a locking gate when it is not open for operation.
2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
3. The facility accepts waste Monday, Tuesday, Thursday, and Friday from 7:30 am until 4:00 pm, and Wednesday
and Saturday from 7:30 am until 12:00 pm.
4. The permit, operations plan, and waste screenings were on -site and reviewed during this inspection. The C&D cover
logs were verified through January 6, 2025.
5. The closed CDLF had been mown. Continue to mow as often as necessary to allow for proper inspection.
6. Overgrown vegetation was observed on the closed MSWLF. The closed MSWLF should be mown as often as
necessary to allow for proper inspection.
7. The asbestos disposal area has been covered with soil and vegetated.
8. Photographs were taken during this inspection.
CDLF Area 2
9. The working face was approaching the '/2 acre size limit. The C&D cover logs were verified through January 6,
2025. Continue to ensure that the waste is covered with a minimum of six inches of soil once weekly or when the
working face reaches % acre in size, whichever occurs first.
10. Mulch had been applied to the side slopes of the CDLF for erosion control and to help establish vegetation. Some
areas of vegetation were observed on the CDLF side slopes. Several areas with erosion rills and exposed waste
were observed near the southeastern corner of the CDLF. Soil had been staged at the top of the CDLF to repair
the erosion rills. Continue to monitor the CDLF Area 2 for erosion and exposed waste and repair any
problem areas as they are observed. Also, as per 15A NCAC 13B .0542 (f) (3), areas that will not have
additional waste placed on them for three months or more, shall be covered and stabilized with
intermediate cover.
11. Some windblown waste was observed on the eastern side slope of the CDLF. Windblown waste was actively being
picked up during this inspection. Continue to ensure that the windblown waste is picked up and returned to the
working face at the end of the day.
12. No unacceptable wastes were identified within the waste stream. Continue to remove unacceptable waste as they
are encountered.
LCID
13. The top of the LCID was being used to store uncovered vegetative waste. The facility has purchased a grinder to
grind C&D waste and the vegetative waste. The grinder is currently being used to grind the vegetative waste on top
of the LCID.
14. A load of yard waste comingled with dimensional lumber was identified in the yard waste area. Mr. Berube
provide the Section with photographs of the waste having been removed and disposed of in the CDLF. Continue
to remove and dispose of unacceptable wastes as they are identified.
15. In accordance with 15A NCAC 13B .0564 (7), the facility must maintain an access roadway around the LCID.
