STATUS OF PERMIT:
6403-LCID-2020: Permit to Construct and Permit to Operate was issued July 13, 2020, and the Permit to Operate will expire
on July 13, 2025. Ensure that the renewal documentation is submitted to Permitting prior to January 13, 2025.
PURPOSE OF SITE VISIT:
Comprehensive Compliance Inspection
STATUS OF PAST VIOLATIONS
None.
OBSERVED VIOLATIONS
None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1. The all-weather roadway had been completed around the LCID. Overgrown vegetation was observed within the
roadway. The access road should be mown and properly maintained.
2. Signage has been posted at the LCID and at the entrance, near the CDLF.
3. Ground vegetative waste from the July 2023 tornado which damaged the Town of Dortches and portions of Nash
County had been windrowed in the western portion of the LCID. The mulch was separated based on location from
which it was received. Ms. Thompson stated that the mulch could be utilized for stabilization purposes around the
facility.
4. No woody vegetation was observed within the LCID footprint. Mr. Frazier stated that he had seeded the LCID
several times, however, the seed had washed away. Ms. Thompson stated that the ground mulch could be helped to
hold the seed in place. It is recommended that the vegetation within the LCID be mown on a regular basis to prohibit
the growth of the vegetation strong enough to puncture vehicle tires.
5. A few pieces of unacceptable wastes were observed within the waste stream. Mr. Frazier stated that the unacceptable
wastes would be removed from the waste stream prior to the covering of the waste.
6. The sediment basin appeared to be functioning properly. Vegetation is growing on the side slopes of the sediment
basin. Continue to monitor and maintain the basin to ensure that the sediment basin is functioning properly.
7. The working face was small and compact.
