SITE PLANADDENDUM
Liquid Containment
Adjacent to the Sorting Pad at a point to be determined by field location a Precast Concrete Catch Basin shall be installed.
The Basin shall be fitted with a Four Inch (4") Lever Operated Butterfly Valve or Emergency Cut Off Valve located in a
separate service port on the outlet side of the catch basin prior to the run of the discharge pipe.
During Operational Hours the valve shall remain in the closed position with an absorbent compound readily accessible to
the basin. During non operational hours the valve shall remain in the open position.

The undersigned, being all of the initial. Members and the initial, Manager of Capitol Waste
Transfer, LLC, a North Carolina limited liability company (the "Company"), do hereby adopt
the following resolutions by signing their written consent hereto:
ELECTION OF MANAGER
RESOLVED, that James M. Barnes is hereby appointed the initial Manager of
the Company to serve as Manager until his resignation, removal, retirement or
death, as the case may be.
OPERATING AGREEMENT
RESOLVED, that the Operating Agreement, attached hereto and identified as
Exhibit A is hereby approved, ratified and adopted in all respects as the Operating
Agreement of the Compa,,y and that a copy be made a part hereof.
CERTIFICATES
RESOLVED, that the form of the share or unit certificate is attached hereto as
Exhibit �B and that such share or unit certificate shall be signed by the Manager of
the Company with the Company seal affixed thereto.
COMPANY SEAL
RESOLVED, that a seal, circular in form and bearing the name "Capitol Waste
Transfer, LLC" on the CIMLIM.Ference and the words "Limited Liability Company
Seal' in the center thereof, an impression of which appears ¢n the margin of this
page, be and the same hereby is approved and adopted as the Company seal of
this Company.
