4. Information about a release;
Upon removal of the UST a release was evident.
B. Site history and characterization;
1. UST owner and operator information;
Johnny and Gail Ferguson
7212 McWhirter Road
Mint Hill, NC 28227
2. Provide UST information;
See the attached table above.
3. Provide non -UST information;
No aboveground tanks were present at the time of the UST removal.
4. Describe relation of location of current UST system to historical
UST releases, non -UST releases, and off -site releases;
There are no documented releases at this site.
5. Site Characteristics;
This site is a private residence. Surrounding property consists of residential
sites. Municipal water and sewer are supplied to the area. The local topography
is relatively flat with some rolling hills and surface drainage flowing to the
South. Groundwater and bedrock were not encountered during excavation
activities. Soils observed at the site ranged from orange to red clay silt. The
property consists of two rental houses each on its own parcel all owned by the
property owner listed
Note: Two home heating oil UST's were present at this site. They were located
in two separate tank pits. Prior to INENCO being on -site the excavation
contractor removed both of these USTs. An initial assessment was performed
by the contractor and INENCO was contacted with that information. The
contractor stated that Tank #1 was discovered to contain @ %2 inch of product,
and that the liquid removed for disposal was fuel/contact water, all from Tank
#2. Also, at the time of removal there was evidence that only one of these
UST's (Tank #2) was involved with a release. The remainder of this report will
only refer to the tank system with the noted release.
UST Closure Report -February, 2024 Ferguson Property
C. Closure procedures;
1. Describe preparations for closure including steps taken to notify authorities,
permits obtained, and steps taken to clean and purge the tank;
As this is a non -regulated heating oil tank no permits were obtained. As stated, the
liquid in the tank was removed. The liquid was then transported to CERT in
Charlotte. The amount of fuel and contact water was 490 gallons. A copy of the
manifests are included. On 1/31/24 the UST liquid was removed for disposal (See
Appendix D).
2. Describe the closure procedures;
The Health and Safety Plan (See Appendix H) was reviewed and signed by all
personnel. On 1/31/2024, soil was removed from the top of the UST to allow for
its removal. Once the tank was removed, soil contamination was encountered.
The tank was heavily pitted, and holes were present. The tank was loaded onto a
trailer to be removed for disposal (See Appendix E). No groundwater, free
product, or bedrock was encountered. See the attached map for sample location
(Figures).
3. Note the amount of residual material pumped from the tank and describe
storage, sampling, and disposal of the residual material and the disposal of
the tanks, pumps, and piping;
Approximately 490 gallons of fuel and contact water were removed prior to the
tank removal. The tank was removed to Titan Metal Co. in Cornelius, NC for
disposal. A copy of the weight ticket (both tanks) and Certificate of Disposal are
included (See Appendix B\E).
4. Document any soil excavation activities;
A back hoe was used for the excavation of the UST's. Soil observed at the site
ranged from orange to red clay silt. The top of the tank was located at the
approximate depth of 2.5 feet. The depth to the bottom of the tank was
approximately 6 feet. Once contamination was encountered the contractor
removed approximately on truck load of contaminated soil. One tank sample from
the bottom of the excavation was collected at a depth of approximately 8 feet.
The dimensions of the excavation were 8 feet wide by 12 feet long by 8 feet deep.
An estimated 10 tons of clean overburden soil was removed to access the tank and
soil contamination. Backfill consisted of retained overburden and additional fill
dirt. No groundwater, free product or bedrock was encountered during the
excavation. The amount of contaminated soil removed was 14.35 tons. A soil
disposal manifest is attached (See Appendix Q.
UST Closure Report -February, 2024 Ferguson Property
D. Site Investigation;
1. Field screening activities;
A Photoionization Detector (PID) is used to field screen soil for any
fuel oil contamination during an excavation. A Rae Systems MiniRAE 2000
photo -ionization detector is used. The MiniRAE 2000 is calibrated to display
concentration in units equal to parts per million. First, a supply of zero air, which
contains no ionizable gases or vapor, is used to set the zero point. Then, span gas,
containing a known concentration of a photo-ioniziable gas or vapor, is used to set
the sensitivity. Isobutylene at 100 PPM is used as the span gas. The field
screening is done by holding the PID next to the soil that had been recently
removed and observing the meter to determine if contaminated soil is present.
In addition, soil samples are placed in plastic bags and allowed to warm and
then read with the PID. The soil samples are removed via use of the back hoe and
taken in native soil at the bottom of the tank as well as from each wall of the tank
cavity with a portion of the soil being split into plastic bags for field checks via the
PID. A strong petroleum odor was noted from the area of the tank. No PID samples
were checked at this location.
2. Document soil sampling information;
INENCO personnel collected a soil sample in the open excavation by use of the
backhoe bucket removing soil from the bottom of the tank area. The tank sample
was collected at a depth of 8 feet below ground level. The soil was placed in new
laboratory provided containers / preparations and taken to the laboratory. See the
attached map for the sample location.
3. Document groundwater and surface water sampling information;
No groundwater or surface water sampling was performed regarding the removal
of this UST. (See Section D.6)
4. Document quality control measure information;
Fresh disposable gloves were used to collect the sample labeled "Tank" and field
prepared it for analysis via Gasoline Range Organics (GRO) and Diesel Range
Organics (DRO) using laboratory provided containers. The prepared sample was
placed on ice in a chilled cooler and taken to the lab. See the attached chain of
custody for the date and time that the sample was submitted to the lab (See
Appendix B).
5. Describe soil and groundwater investigation results;
As described, a single soil sample was collected from beneath the UST. The
sample was collected from about 2 feet below the tank at its estimated center.
The results of the analysis indicated GRO levels of 198 mg/kg and DRO levels
of 6820 mg/kg
UST Closure Report -February, 2024 Ferguson Property
6. Drinking Water Well Survey and Sampling Results;
Prior to the start of the excavation documented in this report, a cursory drinking
water well survey was performed. Except for a very small amount of a neighbor's
front yard, all of the property within 150 feet of the release area is owned by the
property owner. A drinking water well used by a tenant of the property owner
was found to be about 120 feet west of the possible release area. When the
property owner was informed of the situation, the owner asked for a water
sample to be taken from that drinking water well, as soon as possible. On
12/19/2023 the well was sampled. The sample was collected at the well house
spigot after the water was allowed to run for approximately 15 min. The sample
was analyzed via EPA Methods 602 & 625.
The results of the sampling were Non -Detect levels in all constituents. Appendix F
includes a depiction of the location of the well along with the drinking water well
sample results.
E. Conclusions and recommendations;
It was determined that soil contamination did exist at the area of the assessed UST.
No groundwater, bedrock or free product was encountered during the excavation.
The amount of contaminated soil removed was 14.35 tons. Upon the soil removal,
laboratory results indicated that contaminated soil still exists at the site. Additional
remediation items could be required.

S - Surrogate
1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is
a combined concentration.
Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values.
LCS(D) - Laboratory Control Sample (Duplicate)
MS(D) - Matrix Spike (Duplicate)
DUP - Sample Duplicate
RPD - Relative Percent Difference
NC - Not Calculable.
SG - Silica Gel - Clean -Up
U - Indicates the compound was analyzed for, but not detected.
Acid preservation may not be appropriate for 2 Chloroethylvinyl ether.
A separate vial preserved to a pH of 4-5 is recommended in SW846 Chapter 4 for the analysis of Acrolein and Acrylonitrile by EPA
Method 8260.
N-Nitrosodiphenylamine decomposes and cannot be separated from Diphenylamine using Method 8270. The result reported for
each analyte is a combined concentration.
Reported results are not rounded until the final step prior to reporting. Therefore, calculated parameters that are typically reported as
"Total" may vary slightly from the sum of the reported component parameters.
Pace Analytical is TNI accredited. Contact your Pace PM for the current list of accredited analytes.
TN -The NELAC Institute.

Definitions of Sourm
Tank: means the tank that stores the product and is pan of the underground storage.tank system
Piping: means the piping and connectors running from the tank or submersible turbine pump to the dispenser or outer end -use equipment (Vent, vapor recovery, or fill
litres are excluded.)
Dispenser: includes the dispenser and the equipment used to connect the dispense-,' to the piping (e.&, a release from a suction pump or from components located above
the shear valve)
Submersible Turbine Pump (STP) Area includes the submersible turbine pump head (typically located in the ;ink surnp), she line leak detector, and she piping that
connects the submersible turbine pump to the tank
(delivery Problem: identifies releases than Occurred during product delivery to the tank. (Typical causes associated with this source m spills -and overfills.)
Other: serves as the option to use when the release source is known but does not fit into ene of the preced'neg categ[rt'ies (e.g., for releases Poem vent lines, vapor
recovery Lutes, and fill lines)
Unknown: identifies releases for which the source has not been determined
Definitions of Causes
Spill: use this cause when a spill occurs (e.g., when the delivery hose is disconnected from the tank fill pipe or when the nozzle is removed from the dispenser)
Overfill. use when an Ove[fll Occurs (e.g_, overfills may occur from the fill pipe at tine rank or when the nozzle fails to shut off at the dispe-qser)
Physical or Mechanical Damage-. use for all types of physical or mechanical damage; exW corrosion (e.g., puncture of tank or piping, loose finings, broken
components, and components that have changed dimension)
Corrosion: use when a metal tank, piping, ar other component has a release due to corrosion (e.g., for steel, corrosion takes the form of tvst:)
Installation Problem: use when the problem is determined to have occurred specifically because the UST system teas not installed properly
Other- use this gplim when the cause is known but does not $t into one of the preceding categories (evg., putting regulated substances into monitoring we] Is)
Unknown: use when the cause has not been determined

Trudy,
Please find attached a UST Closure Report for the confirmed release of a home heating oil UST at the
subject site. At the direction of the property owner, a limited amount (single truck load) of
contaminated soil was removed from the excavation for disposal.
Please note that the property owner owns two neighboring parcels that are both currently rental
homes. The neighboring parcel includes a water supply well that was measured at a distance of
about 120 feet from the release area described in the attached report. The property owner is in the
process of listing these two parcels together for sale, and has asked me to request a meeting or call
with you once you have a chance to look over the report.
Please let me know when you may be able to discuss.

Hello, Trudy, we removed two 550 gallon home heating oil tanks at the Fairview Road
Location in Mint Hill. The lab results show one of the areas is contaminated. We removed
a total of 14.35 tons of soil that was taken to the disposal facility. Attached are the UST-
61 and lab results for the site. I am working with Ryan Osborne on this site. Please let me
know if you need any further information. Thanks, Larry Boone
