Permit to Operate expired on August 2, 2024. Permit renewal paperwork was submitted on April 24, 2024. A revised Permit to Operate Application was submitted to the Section on February 5, 2021. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: A. March 11, 2019 Permit to Operate Application, Site Design (Page 4, Paragraph 1), "The West Farms Wood Produces wood material treatment and processing facility is in the east central portion of the site and occupies about 6.0 acres (see Total Property Layout & Staging Area — Drawing 2)." PARTIALLY RESOLVED: A revised permit application and permit renewal paperwork have been submitted to Donna Wilson, Permitting Engineer, for review. This violation will be resolved once the new permit application with site plan has been reviewed and approved. OBSERVED VIOLATIONS: None. Page 1 of 3 NORTH CARnLINAD_E Q�� o.n.mm.m m c.w.mmen.i a.i� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. The facility is fenced and is secured by a locking gate when it is not open for operations. 2. The correct signage is located at the entrance of the facility. 3. Operating hours are 7:OOAM to 5:OOPM, Monday through Friday and 7:OOAM to 12:OOPM, Saturday. 4. The facility is permitted to store up to 200,000 tons of treated wood on -site at any one time based on a demonstrated financial responsibility of $229,065.60. 5. The area had been graded to shed water away from the wood stockpiles. No areas of standing water were observed during this inspection. 6. A new swale and sediment basin have been constructed along the southern portion of the operating area. New vegetation was fixable along the swale and sediment basin. 7. The facility was in the process of reorganizing the locations of the windrows to allow for additional space for the Duke laydown yard for new metal electrical poles. 8. The facility was not actively grinding during this inspection. The electrical poles are ground and sent to Virginia. The white wood is ground and sent to McGill for mulching and composting. 9.
