Environmental (operator). Albemarle Regional Health Services is the emergency contact and coordinates additional agencies (fire department(s), etc.) based on the type and severity of the incident. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS:

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any

ADDITIONAL COMMENTS:
1)This facility has a locking gate at roadside entrance for security.
2)Signage has required information. The operator, Albemarle Regional Health Services, is listed along with anemergency contact number. Roads are constructed of all-weather materials.3)Facility is permitted to receive waste from Perquimans, Chowan, and Gates Counties.
4)Waste must be transported to the East Carolina Regional Landfill (0803-MSWLF-1993) in Bertie County.5) Windblown trash was not an issue the day of the inspection.6)No leaks were noted in loaded trailers and troughs are being used to collect any leaking at back of trailers. Theoutgoing loads are being loaded well and tarped.7)The tipping floor and loading bay floor were clean. Some repairs are needed on the concrete at the tippingfloor. Site staff explained that the work is currently under bid status and the facility expects to have a contractor start work on the floor before year’s end. 8) Load-out bay was clean the day of the inspection and appeared to be cleaned daily.9)Screening records reviewed during this comprehensive facility inspection were in compliance. The facility hasgood recordkeeping practices in place.10)Some repairs are still needed in several areas of exterior metal siding.
11)Two (charged) fire extinguishers and a water hose are located on the tipping floor for fire suppression.12) Leachate system appears to be in good working order.
13)Scrap metal area is getting full. All white goods containing CFC are stored upright.
14)Concrete debris area looks good. At the time of a new application, ensure information regarding thecollection of unpainted inert debris is provided, as this may facilitate the addition of this activity in the next permit issuance cycle. 15)The facility has been operating as a Small Type 1 compost facility. Compost piles look good; however, ifthe size of the composting area is greater than two acres, a Large Type 1 permit will be required. Pleasesee information attached to the report and follow-up with the Permitting Unit based on the size of theoperations.
16)Yard waste and tree debris is being managed well, except you MUST maintain the 25ft. fire lane to ensurefirefighting equipment can get around the unprocessed wood and tree debris in the event of a fire. 17)The LCID area appears to be more than one acre and growing. The facility should speak with thePermitting Unit, currently Donna Wilson (donna.wilson@deq.nc.gov), regarding steps involved in an application for a permitted LCID, or discuss plans to close the LCID NC0202 by December 2025. 18)Scrap tires area is being managed well.19)Plastic triple-rinsed pesticide and plastic containers in large plastic bags were being managed well inside a covereddisposal bay.20)Electronics are being collected at this site and are being managed well.
21)Overall management of this facility is good.22) Angel White Transfer Station Management certification expires March 16, 2025.
Certified Operators: David Lowe, expires 5/25/2025, Charlie Rome, expires 2/18/2025, and Johnny Thomas expires March 10, 2026. Stella certified operators: Horace Griffin and Brandyn Drake certifications expire 8/7/2027.
23)Required training is documented. Recommend adding hours and time of classes to the record.

Good morning Autumn. Thank you for taking the time to speak with me Tuesday. It was helpful
for me to talk things through so that I was able to understand how you came to the
conclusions that were included in the report.
Per our discussion, please see the attached information as described below.
Regarding #7 –
The attached 06.11.24 PCG_Meeting_Minutes_REVISED include in section IV, where
Green Engineering presents the overall project to the board for the transfer station
repair and the decision is made to break the project into parts, starting with items 1-4.
The 09.24.24 Minutes from our last PCG Board Meeting. Under section IV you will note
that the board approved the contractor, Watertight Systems, for items 1-4. During the 3
month period between the two meetings the engineering firm created the bid
documents, held the bid meetings, and went through two bid processes due to a lack
of bids received.
I have included the Engineers Probable Cost of Construction which shows items 1-4
(current project) and items 5-7/8 which are the remaining planned projects that the
board already has engineering plans completed for and anticipates moving forward
with after the completion of the first phase. Item #2 is a priority project with the
concrete spall repair in the pit as that poses a safety issue for the operators.
Regarding #17 -
I understand where you are coming from on closing the LCID NO202 from your division
and hopefully we can get the concrete, brick and asphalt moved to the transfer station
permit and “close” that notified site without any issues.
I have included a copy of the original renewal request letter sent in 2019 asking for an
amendment that would move the concrete, asphalt, and brick to the transfer station
permit from the notified LCID NO202. All items were moved/included in the current
transfer station permit under Part VII: Miscellaneous Solid Waste Management, except
the concrete, brick, asphalt. Although I have followed up multiple times, it was never
moved to the permit, not even in the most recent update.
I pulled our map from the transfer station assessment and permit and oddly enough
the concrete area is not marked on the most recent map done by Quible and
Associates. Pulling it up on Google Earth and measuring, the entire area for raw
material, ground material, and our spare can storage is less than 1 acre (see image
below).

Regarding the yard waste management area – as we discussed on the phone, since we
are operating on a closed landfill site, we have a lot of acreage that allows us to be able
to spread out. It looks like a large area, but we have always been careful to keep our
materials to 2 acres. John College made sure to remind us regularly of the rules. One of
the advantages of having the acreage that we have is that we can utilize the grindings
on our acreage without it having to go though the composting process. That allows us
to manage what we have on site and keep the material at an acceptable level to stay
within the parameters of a Small Type 1 facility.
Even though the material is spread out, you can see that the raw/virgin material area is
right at 1 acre and the finished product area is below 1 acre (see images below). The
other area that was mowed in that space is not utilized for yard waste. We do keep it
mowed in order to keep weed seed and vectors from being an issue… plus it just looks
better.
Again, I did pull Google Earth images to measure and show the areas as the Engineer’s
map just has the general area marked.

Thank you for your time this morning to discuss questions regarding items in the inspection
report issued 10/7/2024.
Item 7 – Regarding the tipping floor repair schedule. A correction to the report is being
requested to note the contractor bid process is complete, however, the repair work is
scheduled to be completed in two phases and the first phase includes the push wall and
exterior walls. The second phase includes the tipping floor and apron. Details of the Transfer
Station Repair Project are noted in the County meeting minutes to be provided to the Solid
Waste Section.
Item 17- Regarding the LCID area. A correction to the report is being requested to note the
current notified LCID area is less than one acre and contains inert concrete debris only. The
facility requests the current Transfer Station Permit include a paragraph that covers collection
of (inert concrete debris). The facility does not expect to need a Permitted LCID for the
collection of inert clean concrete. The facility emailed Permitting requesting this during the
last permit renewal in 2023 and is still requesting the inert concrete collection be part of the
Transfer Station Permit. Yard Waste Collection, White Goods, and Scrap Tire are already
included in the 2019 and 2023 permits.
The other issue discussed was the closure of the notified LCID 0202 – This area does not
include yard waste, tree debris, or composting observed on-site the day of the inspection.
It was discussed that a detailed facility site map needs to be reviewed by the Solid Waste
Section to understand the current boundaries and footprint of the compost and yard waste
collection areas to confirm the size of operations observed during the September inspection.
My apologies for any confusion caused by item 7 and 17 in the inspection report.
