The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1)The 5901-MSWLF-1985 inspection has not been done for the past several years. This is the first report
generated and placed under this permit number since 8/24/2016.
2) The facility has entrance signage, directional signage, and locking gate and fences for security.
3) The roads consist of mostly pavement and gravel and were in good condition.
4) The facility has permanent edge-of-waste markers, as required.
5) Landfill gas reports were available and accessible in a notebook onsite, with no issues of note. A few gas wells
that were accessible at the access road front of the closed landfill were observed.
6) Groundwater Monitoring reports were available and accessible in a notebook onsite, with no issues of note. A few
groundwater monitoring wells for the closed landfill were observed. All monitoring wells were locked and had
identification tag with information required. However, I would recommend clearing paths to the monitoring
wells as weather permits, as some wells were not reached the day of the inspection due to recent wet weather
and heavy vegetation.
7) The landfill has a very good cover and is mowed, except where the large trees are present.
8) Approximately four acres of the cap of the original Phase 1 (1985) landfill (closed in 1993) is covered in
large trees. The current facility personnel guessed that the trees are approximately 30 years old or more.
It has been discussed for many inspection report cycles that the damage to the cap to remove trees of that
size and thickness over the four acres would risk the stability of the landfill and the site. No action regarding
the trees was requested at the time of this inspection.
