OBSERVED VIOLATIONS:
1) Permit had expired on 07/14/2022. Talked to and emailed Emily on 12/30/2022 about the expired
permit. Application for permit renewal received by email on 01/04/2023. The paper application was
received by mail on 01/09/2023. Failure to submit the application for permit renewal prior to the
expiration date is a violation of Rule .0840(g) that states "Applications for renewal permits issued in
accordance with this Rule shall be submitted to the Division at least 90 days prior to the expiration date
of the permit. The Division shall notify permit holders of facility permit expiration dates 120 days prior
to permit expiration."
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) The required signage was present.
2) The access road was in good shape.
3) Required markers were present.
4) No septage discharges noticed outside of the permitted boundaries.
5) Applications appeared to follow the nutrient management plan. No spot dumps or thick applications
noticed.
6) No evidence of a winter cover crop (rye or ryegrass). Weed control needed for the broadleaf weeds
(mainly plantain and dandelion).
7) No objectionable odors noticed today.
8) Checked setback distances. Setback distances ok.
9) Permit had expired on 07/07/2022. Talked to and emailed Emily on 12/30/2022 about the expired
permit. Application for permit renewal received by email on 01/04/2023. The paper application was
received by mail on 01/09/2023. Failure to submit the application for permit renewal prior to the
expiration date is a violation of Rule .0840(g).
