The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violationof the Solid Waste Laws, Regulations, Conditions of a Permit
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

2) The pumper vehicle was inspected on 11/17/2022 and failed the inspection because the tank was not
bolted to the frame of the truck. Re -inspected the truck on 111/28/2022 and the tank had been bolted
to the frame of the truck.
3) The required information of Firm name, town name, phone number, and permit number was on both
sides of the pumpervehicle. See pictures below. Each letter of the required information was at least
three (3) inches in height. The lettering was distinguishable and not blocked from view. The lettering
meetsthe requirements of 15A NCAC 13B .0835(b).
4) The tank is made of metal and affixed to the frame of the truck as per 15A NCAC 13B .0835(a)(1).
5) The valves appeared to be in working order and closed as per 15A NCAC 13B .0835(a)(2). Caps present
on ports.
6) Access ports were closed and properly sealed as per 15A NCAC 13B .0835(a)(3).
7) No leaks noticed as per 15A NCAC 13B .0835(a).
8) Truck in compliance with the Septage Rules. Permit issued on 11/30/2022.
