The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) This facility consists of two above ground 15,000-gallon steel tanks for the storage of domestic
septage (including portable toilet waste) and grease septage.
2) "No Trespassing" sign posted at the entrance to Field 3 for SLAS-03-04. This meets the requirement
of Rule 15A NCAC 13B .0838(k).
3) Access road to the tanks is ok and meets the requirement of Rule 15A NCAC 13B .0838(b).
4) Tanks accessible for inspection as required by Rule 15A NCAC 13B .0838(i).
5) No holes or cracks observed in the tanks which may allow the escape of septage out of the tank as
per Rule 15A NCAC 13B .0838(c). No leaks noticed.
6) Septage appears to be transferred to and from the tanks in a manner that prevents leaks or spills of
septage onto the tank or ground as per 15A NCAC 13B .0838(j).
7) No vector or odor issues noticed as covered within Rules 15A NCAC 13B .0838(e)(2) and .0838(g).
Access ports to the tanks are covered.
8) Set-back/buffer requirements appear to have been met.
9) Talked to Aaron about the landowner authorization that is good thru 3/10/2027. Permits now can be
up to 10 years. May want to try for an authorization for a 10 year period.
