OBSERVED VIOLATIONS:
1. Pictures taken by Ms. Amanda Thompson, Environmental Specialist with the Division of Waste
Management— Solid Waste Section, on 10/22/2020 showed a septage spill at this facility. See pictures
below. The septage spill is a violation of Rule .0838(j) that states "Septage shall be transferred to and
from a detention system in a manner that prevents leaks orsoills of seintaae onto the around surface or
exterior surface of the detention system, including septage in pipes used for transferring waste to and
from vehicles."
2. Pictures taken by Ms. Amanda Thompson on 10/22/2020 showed opened ports to some of the buried
tanks. The opened ports are a violation of Rule .0838(e)(3) that states "Each detention and treatment
facilitv shall be desianed, constructed, and maintained to: (3) orevent unauthorized entry into septage
containers or lagoons."
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) On 10/22/2020, Ms. Amanda Thompson, Environmental Specialist with the Division of Waste
Management— Solid Waste Section, performed an inspection of the compost facility, 3506-COMPOST,
which is on the same site as Septage Detention and Treatment Facility, SDTF-35-12. During her
inspection of the compost facility, she noticed some potential issues at SDTF-35-12. See pictures below.
2) On 10/28/2020, Mr. Jeffrey Bullard and I performed and inspection of the Septage Detention and
Treatment Facility, SDTF-35-12.
3) Based on Amanda's pictures and the inspection conducted on 10/28/2020, violations to Rules .0838(j)
and .0838(e)(3) were noted.
4) As noted in "Observed Violations," the septage spill is a violation of Rule .0838(j). By the inspection on
10/28/2020, the septage spill had been cleaned up. See picture below. Thank you.
5) Also noted in "Observed Violations," the opened ports are a violation of Rule .0838(e)(3). The ports
were still opened. See pictures below.
6) The solidification pit for the dewatered septage cake was full of water. See picture below. The water
needs to be pumped out and discharged into one of the septage detention tanks.
7) 1 have scheduled a visit with Mr. Mark Farrell, with Wind River Environmental, on 11/03/2020 to review
the issues noted above.
