The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violationof the Solid Waste Laws, Regulations, Conditions of a Permit
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) As you enterthis site you come to Field 2 first. Field 2 is to the right as you drive north down the path.
Afteryou ga through a wooded area, you come to Fields 1 and 4. Field 1 will be to the right of the path
and Field 4 will be to the left of the path. As you keep going down the path, you will come to Fields 3
and 5. Field 5 will be the furthestfield to the east.
2) Fields labeled as 4 and 5 were previously permitted to A-1 Septic Tank Cleaning. The permit to SLAS-35-
10 was modified to add Fields 4 and 5. Permit Condition 2 required an inspection of Fields 4 and 5 for
crop establishment (fescue) before approval of septage applications to the fields.
3) The required signage was present at the beginning of Field 2 and at Field 4.
4) The access road was in good shape.
5) Required markers were present. Markers were up and visible on all five fields.
6) No septage discharges noticed outside of the permitted boundaries of the fields, in the field borders, or
along the access road to the fields. Fields 2 and 3 have been receiving septage applications.
7) No application issues noticed. No spot dumps, thick applications, or grease burns noticed.
8) Fescue coverage good in all five fields. Septage can now be applied to Fields4 and 5.
9) No objectionable odors noticed today.
10) Setback distances appear to be as permitted. Watch the setback between the septage fields and where
tobacco had been planted. Make that you have a 50 ft separation between the septage field and the
tobacco. I checked a few areas and there was 50 ft separation.
