STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection.
OBSERVED VIOLATIONS:
1) The permit number had an extra "1" and read as "NCS-015117." The permit numberis one of the
lettering requirements of 15A NCAC 13B .0835(b). Please correct the permit numberon both sides of
the pumpervehicle such that it reads as "NCS-01517." Email pictures of both sides of the vehicle
showing the permit number when complete.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violationof the Solid Waste Laws, Regulations, Conditions of a Permit
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

2) The permit number had an extra "1" and read as "NCS-015117." The extra "1" needs to be removed
such that the permit number reads as "NCS-01517." See picture below. Email pictures of both sides of
the vehicle showing the permit number when complete.
3) The firm name, town name, and phone numberwere on both sides of the truck. The letters were three
(3) inches in height. The lettering was distinguishable and not blocked from view.
4) The tank is made of steel and affixed to the frame of the truck as per 15A NCAC 13B .0835(a)(1).
5) The valves appeared to be in working order and closed as per 15A NCAC 13B .0835(a)(2).
6) Tank has no access ports with lids.
7) No leaks noticed as per 15A NCAC 13B .0835(a).
