STATUS OF PERMIT: Initial PTO issued Phase 1, September 6, 2000.
Current PTC C&D Landfill Unit – Cell 2B issued November 15, 2022; PTO MSWLF Landfill Unit – Phases 1, 2, 3, and 4 and C&D Landfill Unit – Cells 1 & 2A issued November 15, 2022.

Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any

ADDITIONAL COMMENTS
1. The facility is fenced and is secured by a locking gate when it is not open for operation. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as
materials accepted at the facility. 2. This Facility is a Municipal Solid Waste Landfill. This Facility is permitted to receive Municipal Solid Waste,
Construction & Demolition Waste, Yard Waste, Land-clearing Debris, and pallets generated in all 100 counties of North Carolina.
3. Facility is currently using posi-shell, C&D fines, automotive shredder residue, biomass ash, and combustion ash as MRF glass residue and C&D MRF fines as alternative daily cover. 4. The facility is permitted to co-dispose of wastewater treatment sludge generated within the facility's approved service area, subject to the terms and procedures of the approved plan and Rule 15ANCAC 13B .1626(1)(e). 5. Random waste screenings records were up-to-date through October 1, 2024. Software randomly picks loads for waste screening. This is done at least two to three times per week. The screening forms note any prohibited items

Groundwater sampling events are being completed semiannually by WSP USA, Inc. First semi-annual groundwater, surface water, and leachate sampling occurred on August 30, 2024.
8. Landfill gas monitoring is being performed quarterly as required. 9. Solidification Mixing Log reviewed and up-to-date through 9/22/2024.
10. The Facility access road is of all-weather construction and is being maintained in overall good condition.
11. Edge of waste markers are in place and easily identifiable around the waste footprint at the facility.
12. Monitoring wells were locked, capped and properly tagged.
13. Windblown waste was being well managed onsite.
14. The intermediate cover at the facility appeared in good condition and well applied. Side slopes are being stabilized with mulch.

15. The waste type in the landfill was reviewed; overall, the waste type being disposed is MSW. 16. The size of the working face was appropriately sized for the expected tonnage. No prohibited or banned items were
observed in the working face. Cover soil was staged near the working face during this inspection. Scent misters were set up on top of the landfill and in operation during this inspection.

17. The leachate collection system cleanouts are highly visible, readily accessible, protected, and well maintained. Solidification pit and surrounding area are being well maintained.

19. The yard waste area looked good at the time of this inspection. There is a container for unacceptable waste. 20. There is at least a 25 foot access path around the site for fire control vehicles. 21. All setbacks are in place.
22. No windblown waste was observed during the inspection. 23. The yard waste is ground into mulch and used on site.
Closed MSWLF 24. The perimeter road around the closed LF should be of all-weather construction and maintained in good condition. 25. The landfill cap is in good condition and is established with adequate ground cover over the majority of the site at
this time.
26. There was no evidence of leachate breakouts at the time of this inspection. 27. The Edge of Waste Markers are in place and easily identifiable around the waste footprint of the facility.
28. Continue to monitor this site for erosion and leachate seepage. 29. Periodically remove any deep root penetrating vegetation that may voluntarily grow on the landfill such as small trees and brush. 30. Digital photos were taken during the inspection.
