Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Pagel of 3 Oepartmenl o� Envimnmental0uali� ADDITIONAL COMMENTS Pumper vehicle(s) inspected: FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section LICENSE PLATE VIN # TANK CAPACITY (gal) YA176933 1FTXR90T4RVA17693 1,250 Comments: N/A 1) 15A NCAC 13B .0835 (a)(1): The tank is constructed of metal and is affixed to the vehicle as required. 2) 15A NCAC 13B .0835 (a)(2): Valves appeared to be in working order and were closed at the time of inspection. 3) 15A NCAC 13B .0835 (a)(3): Access ports were closed and sealed at the time of inspection. 4) 15A NCAC 13B .0835(a): No leaks were noted during the inspection. 5) 15A NCAC 13B .0835 (b): All lettering was at least three (3) inches in height and listed the company name, town name, phone number, and permit number as required. The lettering was permanently affixed as required. At the time of inspection, the lettering was not distinguishable at any angle past 90 degrees due to the metallic material.
