Your application(FID 1895528) has been approved in accordance with NC Septage Management Rules and your permit, SDTF-92-11, is attached. This septage treatment facility consists of one 9,000 gallon, one 15,000 gallon and one 16,500 gallon above ground tanks and includes two dewatering boxes and a bar screen tank. The three tanks have a total capacity of 40,500 gallons for the storage of untreated and treated septage. This permit shall expire on December 31, 2028. An application for permit renewal (including all items in Permit Condition 9) must be submitted ninety (90) days prior to permit expiration. Please read the permit conditions carefully. When communicating to the Division about this permit, refer to it as "SDTF-92-11." Failure to operate the septage detention facility in accordance with NC Septage Management Rules and the enclosed permit could subject you to administrative penalties of up to $15,000 per violation per day. This site is permitted only for operations that are conducted in accordance with the representations made in the approved application, with all conditions attached to this permit, and with all of the provisions of 15A NCAC 1313.0800 -- Septage Management. Failure to operate as permitted may result in the Department suspending or revoking this permit, initiating action to enjoin the unpermitted operation, imposing administrative penalties, or invoking any other remedy as provided in Chapter 130A, Article 1, Part 2 of the North Carolina General Statutes. This facility is subject to periodic inspection, as deemed necessary by the Division of Waste Management, to determine if the facility is in compliance with this permit. This permit is subject to review at least annually, or more often if deemed necessary by the Division of Waste Management, to determine if the operations permitted are consistent with the Septage Management Rules in effect at the time of the review. Modifications to the conditions of this permit may be made where necessary to make this permit conform to the requirements of rules in effect at the time of the review. This septage detention and treatment facility shall be maintained and operated in a manner that will protect the assigned water quality standards of both surface and ground waters. 2. Leaks and spills are the responsibility of the Permittee and shall be immediately contained, cleaned up, and treated to prevent any contamination of the surrounding area. The transfer of septage to and from the detention system shall be conducted in a manner that prevents leaks or spills of septage as stated within Rule 15A NCAC 13B .0838(j). Any liquid or solids collected from the spillage or leakage of septage shall be disposed of in a manner consistent with the NC Solid Waste Management Rules. Major spills shall be reported to the Division of Waste Management within 24 hours of occurrence. 3. The method for controlling odors described in the approved application for this permit must effectively minimize odors created by the operation of this facility as required by 15A NCAC 13B .0838(g). Also, this facility shall be designed, constructed, and maintained in such a manner as required by 15A NCAC 13B .0838(e)(2), to minimize the attraction or admittance of vectors. 4. Erosion and runoff control measures shall be implemented to prevent migration of soil or wastes off this facility. The installation of groundwater monitoring wells or a leak detection system may be required at the discretion of the Division of Waste Management. 5. This permit shall not affect the applicability of any statutes, rules, regulations, or ordinances that may be imposed by other local, state, or federal government agencies that may have jurisdiction over this facility. 6. This permit may be modified or reissued at any time to incorporate any conditions, limitations, and/or monitoring requirements the Division deems necessary to adequately protect the environment and public health. Storage or treatment of any liquid or solid wastes other than grease septage as specified in the approved application for this permit is prohibited. This septage treatment facility consists of one 9,000 gallon, one 15,000 gallon and one 16,500 gallon above ground tanks and includes two dewatering boxes and a bar screen tank. The three tanks have a total capacity of 40,500 gallons for the storage of untreated and treated septage. Liquid and solid waste generated at this facility shall be disposed of in a manner consistent with the approved application and submitted authorizations. Authorizations for liquid and solid waste disposal shall be submitted to the Division prior to utilizing a disposal location. This office shall be contacted immediately if a disposal location revokes the authorization. 8. No one other than the Permittee or an employee of the firm named in this permit may discharge septage at this detention and treatment facility unless the Division is notified and approval is granted by the Division. Only Septage Management Firms permitted to operate in North Carolina may receive authorizations to discharge at this detention and treatment facility. 9. This permit shall expire on December 31, 2028. Renewal or modification of this permit shall be in accordance with 15A NCAC 13B .0837 (i), (m), (n) or (q). An application for permit renewal shall be submitted at least ninety (90) days prior to the permit renewal date. Accurate site drawings (2 copies with a North Carolina PE stamp), a signed & dated operation and maintenance manual (revised to reflect any physical or operational changes at the facility), current solids and wastewater disposal authorizations and a copy of the required facility records shall be submitted with the renewal application. 10.The storage capacity, construction, treatment methods, and operation plan of this facility shall be as represented in the permit application. Any changes shall require a permit modification. 11. Records shall be maintained in accordance with the Septage Management Rule 15A NCAC 13B .0838(v). These records shall be made available to a representative of the Division upon request. A copy of these records for the period of time this permit was valid shall be submitted along with an application for permit renewal or modification. Facility Permit No.: SDTF-92-11 Greasecycle, LLC Permit to Operate September 9, 2024 Page 4 of 4 12.Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises, or place on or related to the facility, at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be kept under the conditions of this permit; or may obtain samples of groundwater, surface water, or leachate. 13.The dewatering boxes shall be cleaned as described in the operation and maintenance manual and in such a manner that all rinsate and solids are contained and disposed of properly. 14.The setbacks prescribed by 15A NCAC 13B .0838 (m) shall be maintained at all times.
