Recorded in Warren County Register of Deeds: Book 0960 — Page 665. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 2 Oepartmenl o� Envimnmental0uali� ADDITIONAL COMMENTS FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 1. As a reminder, based on rule 15A NCAC 13B .0563 (6), the notified LCID landfills will need to be properly closed or obtain a permit from the Division of Waste Management by January 1, 2026. 2. The access road is of all-weather construction and appears to be properly maintained. 3. At the time of this inspection, it appears that the buffer requirements have been met. 4. At the time of this inspection, the northern side slope was nearly vertical and uncovered. The top of the LCID was properly covered with some vegetation. As per 15A NCAC 13B .0566 (3), ensure that all waste is covered with a minimum of six inches of soi monthly or when the working face reaches one acre in size, whichever occurs first. 5. No unacceptable wastes were observed within the waste stream during this inspection. 6. Some cover soil has been stockpiled on top of the LCID. 7. Digital pictures were taken during the inspection.
