Life of Site, issued on December 20, 2023, and expires on October 08, 2053 PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: Resolved OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 9 ADDITIONAL COMMENTS: 01. This site visit was for a comprehensive facility compliance inspection. 02. This permit approves the construction of Phases 6 and 7 Cells 16 and 17. The initial, substantial, construction authorized by this Permit Approval to Construct must commence within 18 months from the issuance date of the permit issued December 20, 2023. If substantial construction does not begin by this date, then the permit to construct shall expire. 03. This permit to operate covers Phases 1-5 and Phase 6- Cell 15. 04. The Facility has locking gates and fencing for security. 05. The Facility has adequate signage. 06. The Interior roads were in good condition. Roads were wet from morning rains, so no dust control was needed at the time of the inspection. 07. Plans are ongoing after large rain events like this morning, for soil to be added to cells with some exposed waste and some erosion. It was explained that due to the soils type at the landfill that rain events require consistent maintenance. 08. A decent supply of cover material (soil) is being staged at the working face for quick and adequate cover. 09. Tarps are also being used as daily cover. 10. Edge of waste markers are present. 11. No Auto Residuals are being used at this site. The facility has a contract for receipt of Auto Residuals, but it was reported that none have been received. 12. The working face was large. 13. The facility holds an NCG010000, and NCG12000 stormwater permits under NC DEMLR. 14. The facility holds an air permit under NC DAQ. 15. The facility has a landfill gas collection system, and a gas plant is being constructed at the site that is a joint venture between Arcada and Dominion Energy. Gas monitoring records were available, but not reviewed. 16. No new leachate breakouts were observed leaving the landfill footprint during this inspection. 17. Leachate is pumped and collected into a large storage tank farm in secondary containment. The facility holds a current Pump and Haul Permit and disposal agreement with the City of Rocky Mount Wastewater Treatment Plant for transport and disposal of the leachate. 18. Some areas that have been repaired on the side slopes have a decent amount of vegetation growth. Some slopes need vegetation. The facility does a good job collecting and controlling windblown trash with very minimal amounts observed during this inspection. 20. The facility has Shannon Hunter certified NC SWANA Landfill Operations Manager, expires (exp.) 3/14/2025, Michael Bazemore certified NC SWANA Landfill Operations Specialist, exp. 11/15/2025, Luis Diaz certified NC SWANA Landfill Operations Specialist exp. 7/13/2025, Mitch Hoggard certified NC SWANA Landfill Operations Specialist exp.11/15/25, Jesse Robertson NC SWANA Landfill Specialist exp.7/13/25 and Robert Patterson, NC SWANA Landfill Specialist exp. 7/13/25. 21. Records Reviewed – current Permit and Operations Plan copies were available and discussed. The facility should update the operations plan dated 2019, as needed, and I would recommend documenting an annual review of the operations plan to show the plan is being kept up to date as required. 22. Screening records, weight tickets, and daily compaction records were available, and reviewed and were up to date. 23. Training . It is recommended as part of the training that the facility provides training on the Solid Waste Permit and the Facility Operations Plan. 24. Overall, this facility is being managed well. 25.
