Permit will expire on September 24, 2026. Life of Site Permits issued for 4302-CDLF-1998 and 4302-CDLF-2018 on July 23, 2020. The Life of Site Permits will expire on March 2, 2058. PURPOSE OF SITE VISIT: Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None. OBSERVED VIOLATIONS: None. Page 1 of 4 NORTH CARnLINAD_E Q�� o.n.mm.m m c.w.mmen.i a.i� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS Facility Inspection 1. The facility is fenced and secured by a locking gate when it is not open for operation. 2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 3. The facility accepts waste Monday, Tuesday, Thursday, and Friday from 7:30 am until 4:00 pm, and Wednesday and Saturday from 7:30 am until 12:00 pm. 4. The C&D cover log was verified through July 24, 2024. 5. Overgrown and woody vegetation was observed throughout the facility. Mr. Berube stated that heavy rains and a broken mower had stopped the mowing progress at the facility. Continue to mow the facility after the mower is repaired and the soils have dried to allow for proper inspection and to minimize damage to the landfill caps. CDLF Area 2 6. Two working face areas, one near the southwestern corner and one at the top of the CDLF, were identified during the July 25, 2024 inspection. The total working face area was approaching the %2 acre size limit during the July 25, 2024 inspection. During the July 25, 2024 inspection, Ms. Thompson stated that the working face needed to be covered and a follow-up inspection to measure the size of the working face. During the July 30, 2024, the uncovered working face was measured to be 0.34 acres in size. Continue to ensure that the waste is covered with a minimum of six inches of soil once weekly or when the working face reaches V2 acre in size, whichever occurs first. 7. Mulch had been applied to the side slopes of the CDLF for erosion control and to help establish vegetation. Some areas of vegetation were observed on the CDLF side slopes. Several areas with erosion rills and exposed waste were observed near the southeastern corner of the CDLF. Soil had been staged at the top of the CDLF to repair the erosion rills. Continue to monitor the CDLF Area 2 for erosion and exposed waste and repair any problem areas as they are observed. Also, as per 15A NCAC 13B .0542 (f) (3), areas that will not have additional waste placed on them for three months or more, shall be covered and stabilized with intermediate cover. 8. A wind screen has been installed along the eastern side slope to help catch windblown waste. 9. During both inspections, several tires were observed within the waste stream. Mr. Beane and the equipment operator removed the tires from the waste stream. Ensure that unacceptable wastes are removed from the waste stream as they are identified. 10. The facility submitted the CQA report for the Phase 2A extension to the Section on July 9, 2024 for review. No waste can be placed beyond the approved berm without the approval of the CQA report for Phase 2A from Permitting. 11. The sediment basin for the Phase 2A extension has been installed. Seed and straw matting has been placed on the side slopes of the sediment basin. LCID 12. The top of the LCID was being used to store uncovered vegetative waste. The land clearing waste was actively being ground during the July 30, 2024 inspection. 13. Waste containers have been placed on the LCID so that the drivers can place unacceptable wastes in them for proper disposal. Mr. Beane stated that the City truck drivers have been advised that they are responsible for removing unacceptable wastes. Any additional unacceptable wastes identified in the waste stream are removed by the equipment operator for proper disposal. In accordance with 15A NCAC 13B .0564 (7), the facility must maintain an access roadway around the LCID. 15. Photographs were taken during the July 25, 2024 inspection.
