The Solid Waste Section approved Duke Energy's request for abandoning and replacing Industrial Landfill
No.1 (1812-INDUS; ILF) Phase 3 piezometers CLF-22-306 and CLF-22-307 in a letter dated June 14, 2024.
Duke Energy is providing the Completion Report for the piezometer replacements, including the well
abandonment records, well installation records, boring logs, well construction information, survey data, and
water level information both pre -abandonment and post -installation. The piezometer replacement activities
were conducted by S&ME on June 25-27, 2024.
Well abandonment and construction records (GW-30 and GW-1 forms) were submitted separately to the
NCDEQ Information Processing Unit.

Dear Ms. Healy:
S&ME, Inc. (S&ME) is pleased to present this Completion Report (hereafter, Report) for the replacement of two (2)
groundwater monitoring wells at Duke Energy's Marshall Steam Station (hereafter, Site), located at 8320 NC Hwy
150 E, in Catawba County, North Carolina. These wells were replaced to support the construction of Cell 7 within
Phase 3 of the Industrial Landfill (ILF) — Permit No. 1812. This Report summarizes the activities performed by
S&ME at the Site between June 2511 and 27t", 2024, as well as a certification statement signed by a North Carolina
licensed geologist, Julie Robertson, P.G, certifying that each monitoring well was abandoned and replaced in
compliance with the applicable standards and guidance as discussed in this Report.
Background and Summary
S&ME understands that as part of the ILF Phase 3 geotechnical investigation, monitoring wells CLF22-306 and
CLF22-307 were installed by others in March of 2022 to collect monthly groundwater elevation data and
demonstrate the vertical separation between groundwater and the compacted soil liner base grade design for Cell
7. Duke Energy requested that these wells be relocated due to their current location being in the vicinity of haul
roads and ongoing construction and earthwork activities. Additionally, the water levels in CLF22-306 and CLF22-
307 have declined several feet since they were installed.
To continue monitoring groundwater levels and confirm the groundwater and soil base grade vertical separation
is met, replacement wells (CLF22-306R and CLF22-307R) were installed as close as practical to the original wells,
given the current and future Site conditions and on -going construction activities (see Figure 1 in Attachment 1).
These replacement wells were also installed deeper than the original wells to allow for an increased water column
in each well for continued groundwater elevation data collection.
Monitoring Well Abandonment
The original monitoring wells CLF22-306 and CLF22-307 were abandoned by a North Carolina registered well
driller on June 261", 2024. Each monitoring well was abandoned in accordance with the 75A NCAC 02C Section

.0113 Standards for the Abandonment of Wells (hereafter, Well Abandonment Standards). S&ME personnel
provided support and oversight during the abandonment process for each monitoring well to confirm compliance
with the Well Abandonment Standards as mentioned above.
Per the Well Abandonment Standards, a well must be permanently abandoned if it acts as a source or channel of
contamination. Each of these wells were abandoned prior to earthwork and grading operations to reduce the
likelihood that the wells would act as a channel for contaminants to reach groundwater once the well was
removed during excavation and earthwork activities.
A summary of pertinent monitoring well information for each of the wells abandoned is located in Table 1 in
Attachment II. As required by the Well Abandonment Standards, a Well Abandonment Record (GW-30 Form)
was completed and signed for the two (2) wells abandoned. S&ME submitted these GW-30 Forms to the North
Carolina Department of Environmental Quality (NCDEQ) — Division of Water Resources (DWR) Information
Processing Unit, and they are included in Attachment III as reference.
Each of the monitoring wells were abandoned following the general steps listed below:
If present, any dedicated tubing, pumps, or transducers were removed from the well and left on site with Duke
Energy personnel.
Depth to water from ground surface was measured using an electronic water level meter. The bottom of each
well was also tagged to ensure no discrepancies between recorded and actual well depth data exist.
The volume of water in the well was calculated based on the measured water level.
Each well was abandoned using coated bentonite pellets in accordance with the general requirements of 02C
Standards, as equipment access to these wells was very limited at the time of abandonment.
S&ME understands that the protective well covers and concrete pads will be removed from each of the
abandoned well locations by the general earthwork contractor on Site.
Installation of Replacement Monitoring Wells
The replacement monitoring wells were installed by a North Carolina Registered well driller; CLF22-306R was
installed on June 251", 2024, and CLF22-306R was installed on June 261", 2024. Each of these wells was constructed
as typical Type II Monitoring Wells in accordance with the 75A NCAC 02C Section .0108 Standards of Construction:
Wells Other than Water Supply (hereafter, Well Construction Standards). S&ME personnel provided support and
oversight during the well installation process.
Each of the replacement monitoring wells were installed following the general steps listed below:
A borehole was drilled using 3.25-inch Hollow Stem Augers (HSAs) to the target elevations provided by Duke
Energy. It should be noted that CLF22-307R was drilled 3.2-feet deeper than proposed to confirm adequate
water would be present once the well was installed. No sampling was performed; however, S&ME personnel
classified soil from the auger cuttings in general accordance with the United Soil Classification System (USCS),
in addition to recording other pertinent drilling information including hard drilling zones and the depth water
was encountered during drilling.
The HSAs were removed from the borehole and a 2-inch diameter Type II monitoring well was installed at
each location (CLF22-306R and CLF22-307R) as specified below:

Once well installation was completed, each well was developed by S&ME personnel on June 261" and 27t", 2024,
following the general steps below:
Depth to water was measured (from ground surface), and the volume of water present in each well was
calculated
Each well was then pumped until a minimum of three (3) well volumes were removed, and a turbidity less
than 50 NTUs was achieved
If a well was pumped dry, it was monitored, and recharge rates were recorded
Periodically, the pump was moved up and down to increase agitation and removal of any soil particles
during development
Boring logs including the soil lithology, drilling observations, and well construction details are included as
Attachment IV. A summary of pertinent monitoring well construction information for each of replacement wells is
located in Table 2 in Attachment II. As required by the Well Construction Standards, a Well Construction Record
(GW-1 Form) was completed and signed for the two (2) wells installed. S&ME submitted these GW-1 Forms to the
NCDEQ-DWR Information Processing Unit, and they are included in Attachment III as reference. Well
development forms are included in Attachment V.
Monitoring Well Replacement Certification
Monitoring wells CLF22-306 and CLF22-307 discussed in this Report were abandoned in accordance with the
requirements of the Well Abandonment Standards by a certified Well Driller licensed in the State of North
Carolina. Replacement monitoring wells CLF22-306R and CLF22-307R were installed as typical Type II monitoring
wells by a certified Well Driller licensed in the State of North Carolina. Under the supervision of an S&ME licensed
geologist, an S&ME professional was on site to observe each well abandonment and installation.
