Duke Energy is hereby requesting approval to abandon the wells associated with the SSLF dewatering system
that are within the SSLF footprint. The wells planned for abandonment are shown on the attached figure. I have
placed a red box around the symbols in the legend that represent wells associated with the SSLF dewatering
system and are part of the current planned abandonments. The eductor (extraction) wells will be abandoned per
15A NCAC 02C.0113. Specifically, the wells are planned to be abandoned via in -place tremie methods using a
flexible grout that qualifies as an allowable grout per the aforementioned regulation. As required by the SSLF
Permit (3620-INDUS-2022), a Licensed Geologist will oversee and certify the abandonments and copies of the
GW-30 Well Abandonment Forms will be submitted to the Section.
The vibrating wire piezometers (VWPZs) that are associated with the dewatering well system are already fully
grouted below ground surface by design, so abandonment of those locations will simply comprise removing their
surface instrumentation.
For reference, I've attached copies of the respective NCDEQ DWR Recovery Well Construction Permits (original
for pilot section and modified for full-scale expansion), as well as copies of the GW-1 Well Construction Forms for
the eductorwells.
The site and contractors are prepared to begin abandoning the wells associated with the SSLF dewatering
system within the next week or two, pending your approval.

In accordance with your completed permit modification application received on June 2, 2023, we are forwarding
herewith Water Supply Well Construction Permit No. WS0300028 dated June 2, 2023, issued to Duke Energy, for the
construction of a water supply well system with an increased design capacity of 290,000 gallons per day.
This Construction Permit covers the installation of an additional fifty-three (53) dewatering wells not to exceed a depth
of 70 feet below land surface (bls).
This Permit will be effective for one year from the date of issuance and shall be subject to the conditions and
limitations as specified therein.
Duke Energy should also verify with DWR's Complex Permitting Unit regarding coverage of the proposed additional
discharges to the active ash basin at the facility under NPDES Permit NC0004987.
Please note that, depending upon the volume of water withdrawn by the system per day, registration of the
withdrawal may be necessary at: https://www.ncwater.org/WUDC/app/WWATR/page.

(Wells or Well Systems with a design capacity of 100,000 Gallons Per Day or Greater)
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and other applicable Laws,
Rules, and Regulations.
PERMISSION IS HEREBY GRANTED TO
Duke Energy Carolinas, LLC
FOR THE CONSTRUCTION OF A WATER SUPPLY WELL SYSTEM consisting of fifty three (53) additional dewatering
wells owned by DUKE ENERGY and located at the subject site at 253 Plant Allen Road, Belmont, North Carolina. This
Permit is issued in accordance with the completed permit modification application received on June 2, 2023, in conformity
with specifications and supporting data, all of which are filed with the Department of Environmental Quality and are
considered integral parts of this Permit.
This Permit is for well construction only, and does not waive any provision or requirement of any other applicable law or
regulation. This Permit does not constitute approval of a site, for which the Public Water Supply Section may require
additional information or approval of the proposed volume of water to be withdrawn, within an area defined as a Capacity
Use Area, regulated by the Division of Water Resources.
Construction of any well under this permit shall be in strict compliance with the North Carolina Well Construction Regulations
and Standards (15A NCAC 02c .100), and other State and Local Laws and regulations pertaining to water supply well
construction.
If any requirements or limitations specified in this Permit are unacceptable, you have a right to an adjudicatory hearing upon
written request within 30 days of receipt of this Permit. The request must be in the form of a written petition conforming to
Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made, this Permit is final and binding.
This Permit will be effective for one year from the date of its issuance and shall be subject to other specified conditions,
limitations, or exceptions as follows:
1. A well test for yield shall be conducted in accordance with 15A NCAC 02C .110(b) and the results of that test shall be
submitted to the Division of Water Resource's Mooresville Regional Office within 30 days of the test conclusions.
2. Each well shall have a Well Contractor Identification Plate and a Pump Installation Information Plate in accordance
with 15A NCAC 02C .0107Q) (2) and (3).
3. Well construction records (GW-1) for each well shall be submitted to the Division of Water Resources' Information
Processing Unit within 30 days of the well completion.
4. When the well is discontinued or abandoned, it shall be abandoned in accordance with 15A NCAC 02C .0113 and a
well abandonment record (GW-30) shall be submitted to the Division of Water Resources' Information Processing
Unit within 30 days of the well abandonment.
