PURPOSE OF SITE VISIT:
Follow-up to a Fire and Partial Compliance Inspection

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

On July 17, 2024, Mr. Randy Deming contacted Ms. Thompson to report that a fire had occurred in a transfer trailer during
loading. Upon noticing the smoke, the operator used a hose to saturate the trailer before removing the trailer from the loading
bay and calling the fire department. Flames were observed in the trailer once outside the building. The Vance County Fire
Department extinguished the fire. The water used to extinguish the fire was contained within the leachate collection system.
The leachate collection system was pumped following the fire.
1. The facility is fenced and is secured by a locking gate when it is not open for operation.
2. The correct signage is at the entrance of the facility listing the facility name, permit number, and emergency contact
information as well as material accepted at the facility.
3. The facility accepts waste Monday through Friday 0530 to 0700 for private operators, Monday through Friday 0730
to 1630 for the public, and Saturday 0730 to 1200.
4. The permit, operations plan, waste screening, and training records were not reviewed during this inspection.
5. Waste was observed on top of the push walls. Mr. Murray stated that the waste would be removed from the push
wall at the end of the operating day. Continue to ensure that the waste on top of the push walls is removed and
properly disposed of at the end of the operating day.
6. Waste was observed within the transfer trailer loading bay. Mr. Murray stated that the waste is removed from the
loading bay as needed throughout the day. Ms. Thompson stated that it appears that the loading by has not been
cleaned in a while. Ms. Thompson recommended cleaning the transfer trailer loading bay more frequently to assist
in managing the windblown litter and to ensure that the leachate drains are functioning properly. Mr. Murray
provided the Section with photographs of the cleaned transfer trailer loading bay at the end of the day. Ensure that
the waste is removed from the transfer trailer loading bay as needed throughout the day and at the end of
the operating day.
7. Minimal windblown waste was observed in the grasses around the facility. Mr. Murray stated that the facility has a
dedicated person to pick up windblown waste throughout the operating day. Continue to ensure that the
windblown waste is picked up prior to the end of the operating day.
8. Waste was observed around a roll -off container located to the south of the transfer trailer loading bay. Mr. Murray
provided the Section with a photograph of the waste removed from around the roll -off prior to the end of the
operating day.
9. An area of standing water and mud was observed near the southern end of the transfer trailer loading bay. Mr.
Murray stated that the area of standing water was stormwater and the result of the recent heavy rains. The area
should be graded to prevent ponding near the transfer trailer loading bay.
10. A new concrete retaining wall and push plates have been installed at the southern end of the transfer trailer loading
bay which assist in the removal of the waste from the loading bay.
11. A new concrete drainage ditch has been poured along the haul road to help prevent erosion of the haul road.
12. Digital photographs were taken during the inspection.
