Shackelford, NCDENR - Solid Waste Section Brian Polk, NCDENR — Safety Consultant Chester Bryant, Owner STATUS OF PERMIT: Permit Issued — April 16, 2001 (expires April 16, 2006) PURPOSE OF AUDIT: Comprehensive Audit NOTICE OF VIOLATION: None You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. STATUS OF PAST NOTED VIOLATIONS: None FACILITY COMPLIANCE AUDIT REPORT Division of Waste Management Solid Waste Section Page 2 of 2 OTHER COMMENTS /SUGGESTIONS: 1. The waste type being accepted at the facility is according to permit. 2. Other waste types have been removed from the LCID Landfill. The waste piled up that was removed previously needs to be placed into an approved container or removed from the area and properly disposed of weekly. 3. The exposed area was less than one acre and is being covered as required. Previously covered areas need to be provided with adequate ground cover sufficient to restrain erosion within 30 days. 4. Chipping of land clearing debris is being done today. There were several piles of chipped land clearing debris. The site is being managed well. Good Job! 5. Pictures were taken today for file.
