NOTICE OF VIOLATION: None
You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each
violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions
including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such
further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

OTHER COMMENTS /SUGGESTIONS:
1. The waste at the toe of the slope and adjacent areas that is outside the limits of the landfill need to be
removed and returned to the working face within 2 days of receipt of this notice. Waste outside the limits of
the landfill needs to be picked up at the end of each day.
2. I am in receipt of the August 12, 2003 letter written to Jim Barber concerning the renewal of this permit. Efforts to
renew this permit need to continue.
3. The on -site erosion needs to be eliminated. Disturbed areas need to be provided with adequate ground cover
between any phase of grading that is inactive 30 days or more. As a minimum, the stockpiles and slopes of the
C&D landfill that are eroding need to be properly graded, seeded and mulched.
